This is to certify that any people named on this form are attending the TN Gurmat Camp at Fall Creek Falls State Park with consent.

I do, hereby, for myself, my children, heirs and executors forever waive and release TN Gurmat Camp, its employees, volunteers, and agents from any and all claims, actions, demands, rights and damages of any nature whatsoever, that I or my children may have at any time against TN Gurmat Camp, its directors, officers, employees and agents.

Additionally, I hereby grant to TN Gurmat Camp the right to photograph any volunteers registered and use the photo, film and or other digital reproduction of them for publication purposes, whether electronic, print, and digital or internet publishing. I certify that I am a custodial parent and have the aforementioned rights to assign.

At Tennessee Gurmat Camp, we prioritize creating an environment that fosters meaningful connections with sangat, immersive experiences, and personal growth for our campers. To ensure that every camper can fully engage in camp, we have established the following cell phone policy:

1. Restricted Usage: Cell phones are not permitted during certain parts of the day, including Gurdwara divaan, classes, and evening activities. People are required to leave their phones in their dormitories during these designated times.
2. Permissible Usage: People may use their cell phones solely during sports time or excursions, such as trips to waterfalls, where they may need them for photography, communication, or safety purposes.
3. Lights Out Policy: In consideration of others, cell phone usage is prohibited after lights out at nighttime. If someone needs to use their phone as an alarm clock, they are reminded not to set multiple repetitive alarms to prevent unnecessary disturbance to fellow campers.
4. Disclaimer: The camp administration holds no responsibility for any damage, loss, or theft of cell phones. It is your responsibility to ensure the safety and security of their personal belongings.

By adhering to this policy, we aim to cultivate an environment where campers can fully immerse themselves in the camp experience, build lasting friendships, and embrace the opportunities for growth and discovery. We appreciate the cooperation of campers and their families in upholding these guidelines.

Failure to adhere to this policy may result in the confiscation of devices by camp sevadaars.

I, the undersigned, do hereby authorize Tennessee Gurmat Camp as agent(s) for the undersigned to consent to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by and is to be rendered under the general or special supervision of any physician and surgeon licensed under the provisions of the Medical practice Act on the medical staff of any accredited hospital, where such diagnosis or treatment is rendered at the office of said physician or at said hospital. It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care which the aforementioned physician in the exercise of his/her best judgment may deem advisable.

I further agree to be responsible for all costs associated with any treatment provided in compliance or attempted compliance in good faith with this consent.
